Complete Intake Forms
After booking, you'll receive a link to your Patient Intake Form and Consent documents. Complete these before your appointment so your doctor is ready for you.
See Your Doctor
At your scheduled time, join your private call through Doxy.me — audio by default, video if your doctor requests it. No app download needed.
Receive Your Care Plan
Your doctor sends prescriptions to your pharmacy, orders labs if needed, and follows up via message. You'll have everything in writing.

Choose a time that works for you. Payment of $59 is collected at booking. After confirming, you will receive your Patient Intake Form and Consent documents by email. Join the link at the time of your appointment and see your doctor.
As required by the federal No Surprises Act, self-pay patients receive a written Good Faith Estimate of expected charges by email after booking. The standard visit fee is $59; labs and prescriptions, if needed, are billed separately by the lab or pharmacy.
