Patient Rights & Responsibilities

Crescent Health is committed to providing dignified, respectful, and high-quality telehealth care. As a patient, you have the following rights and responsibilities.
Your Rights as a Patient
- Right to Respectful Care: You have the right to receive care that is considerate, respectful, and delivered with recognition of your personal dignity, regardless of race, color, national origin, sex, age, religion, disability, or sexual orientation.
- Right to Informed Consent: You have the right to receive a clear explanation of your diagnosis, proposed treatment plan, risks, benefits, and alternatives — in language you can understand — before consenting to care.
- Right to Refuse Treatment: You have the right to refuse any recommended treatment or service and to be informed of the potential medical consequences of that decision.
- Right to Privacy and Confidentiality: You have the right to have your medical information kept private and secure, consistent with HIPAA and applicable state law. See our Privacy Policy for full details.
- Right to Access Medical Records: You have the right to access, review, and obtain a copy of your medical records. Submit a request to admin@crescent-telemed.com.
- Right to Amend Records: You have the right to request corrections to inaccurate or incomplete information in your health record.
- Right to Know Your Care Team: You have the right to know the name, credentials, and role of every provider involved in your care.
- Right to Transparent Pricing: You have the right to know the cost of services before your visit. Our standard self-pay rate is $59 per visit with no hidden fees.
- Right to File a Complaint: You have the right to voice concerns or file a formal grievance without fear of retaliation or negative impact on your care.
- Right to Continuity of Care: You have the right to receive information about follow-up care, referrals, and alternative services when Crescent Health cannot address your needs.
Your Responsibilities as a Patient
- Provide Accurate Information: You are responsible for providing honest and complete information about your medical history, current symptoms, medications, and allergies. Inaccurate information may affect the quality of care you receive.
- Complete Intake Forms: You are responsible for completing the Patient Intake Form and Consent documents prior to your appointment.
- Follow the Care Plan: You are responsible for following the treatment plan agreed upon with your provider and for communicating any concerns if you are unable to do so.
- Keep Appointments: You are responsible for attending your scheduled appointment on time or cancelling with adequate notice (at least 24 hours in advance).
- Seek Emergency Care When Needed: You are responsible for calling 911 or going to an emergency room if you are experiencing a medical emergency. Crescent Health is not an emergency service.
- Respectful Conduct: You are responsible for treating our providers and staff with courtesy and respect. Abusive, threatening, or inappropriate behavior may result in termination of care.
- Payment: You are responsible for paying the applicable service fee at the time of booking.
How to File a Grievance
If you have a complaint or concern about your care or your rights as a patient, you may:
- Email us at admin@crescent-telemed.com — we will acknowledge your complaint within 2 business days and provide a written response within 7 business days.
- File a complaint with the Georgia Composite Medical Board: gcmb.georgia.gov
- File a HIPAA complaint with the U.S. Department of Health and Human Services Office for Civil Rights: hhs.gov/ocr/complaints
